Unity House Case Manager Young Adult/MICA in Troy, New York

Please consider joining Unity House of Troy's CHAP program as a Case Manager who assists Rensselaer County's young adult populations.

Position may be eligible for a $750 sign on bonus

Responsibilities:

Engage in ongoing, regular interactions with clients.

May need to provide or assist with crisis prevention and intervention.

Assist clients with identifying and obtaining goals for education/employment.

Provide transportation to appointments as needed.

Assist with making referrals for services.

Advocate on the client’s behalf with outside agencies.

May need to provide home and field visits.

Provide daily living skills training.

Management of symptoms.

Assist with social development.

Promote an environment of inclusion and belonging.

Required:

Bachelor’s or Associate’s degree in health, human services with one-year experience with multiple populations preferred. High school/GED required with two-years experience. Valid and current New York State Driver License is required. Must have reliable transportation. Bi-lingual candidates encouraged to apply.
